PROCEDURAL PATHWAY AT-A-GLANCE
Indications, Surgical Technique & Recovery
1. WHO IT'S FOR
• Benign or radiation-induced mid/upper ureteral strictures • Chronic hydronephrosis causing renal compromise • Failed endoscopic ureteral stent or balloon dilation
2. HOW IT'S DONE
• Robotic-assisted DaVinci laparoscopic reconstruction • Buccal mucosal graft (BMG) dorsal onlay augmentation • Firefly (ICG) fluorescence imaging for tissue perfusion
3. RECOVERY
• Hospital stay: 1 night (observation) • Internal double-J ureteral stent for 4–6 weeks • MAG3 renal scan confirmation of drainage
Advanced reconstruction of the ureter to repair high-pressure narrowings or scar barriers. Utilizing precise tissue mobilization, structural configurations, or robotic-assisted approaches, this procedure preserves long-term renal function by ensuring clear, uninhibited urine transit from the kidney.
Clinical Indication
This procedure is highly indicated for patients experiencing structural anomalies, severe localized scar tissue, or functional blockages that fail to resolve with initial conservative therapeutic adjustments.
Surgical Objective
The principal objective is the complete restoration of physiological form, functional urinary path independence, or mechanical integrity, maximizing long-term outcomes while minimizing systemic downtime.
Ureteroplasty represents the highest tier of minimally invasive reconstructive urology, restoring wide, unobstructed urinary transport between the renal pelvis and bladder without the lifetime burden of chronic indwelling ureteral stents. While repetitive balloon dilation or laser endoureterotomy for mid and proximal ureteral strictures often fails over time, while robotic ureteroplasty with a buccal graft or primary repair offers a more durable result with a shorter recovery than open surgery.
Clinical Indications & Patient Selection
Indicated for benign or radiation-induced ureteral strictures resulting from prior ureteroscopy, kidney stone impaction, pelvic surgery, or endometriosis.
Persistent Hydronephrosis & Flank Pain: Progressive renal collecting system dilation threatening irreversible renal parenchymal nephron loss.
Stent Dependence or Stent Intolerance: Patients suffering from chronic severe bladder spasms, hematuria, and frequent stent exchanges every 3 to 6 months.
Failed Endoscopic Dilation: Strictures longer than 1 cm or dense devascularized ischemic scar tissue unresponsive to endoureterotomy.
Surgical Technique & Procedural Nuance
Performed via the DaVinci robotic platform utilizing real-time near-infrared fluorescence (Firefly™) with Indocyanine Green (ICG) to assess microvascular ureteral perfusion.
Robotic Mobilization & Stricturotomy: Meticulous ureteral mobilization preserving the adventitial blood supply, followed by longitudinal incising of the strictured segment until healthy pink mucosa is reached.
Buccal Mucosa Graft (BMG) Onlay: A customized inner cheek oral mucosal graft is introduced into the abdomen, sutured as a dorsal or ventral onlay patch using running 4-0 absorbable sutures, and stented over a 6Fr double-J catheter.
Omental / Peritoneal Flap Wrap: The reconstructed ureteral segment is wrapped with a vascularized pedicle of omentum to promote rapid graft inosculation and prevent retroperitoneal fibrosis adhesions.
Postoperative Pathway & Recovery Timeline
Conducted with an overnight hospital stay, with patients mobilized on the day of surgery.
Internal Double-J Stent: Maintained for 4 to 6 weeks to protect the graft while vascularization and re-epithelialization occur.
Stent Removal in Clinic: Removed via a gentle, 2-minute flexible cystoscopy in the office setting under local lidocaine gel.
Functional Follow-Up: MAG3 Lasix renogram drainage scan performed at 3 months confirms complete resolution of kidney obstruction.
Outcomes, Durability & Clinical Evidence
Robotic BMG ureteroplasty offers lasting relief from ureteral obstruction while avoiding complex open flank surgeries.
Success Rate: Most patients have a lasting result in published multi-center series.
Preservation of Kidney Function: Halts progressive nephron loss and completely eliminates dependence on indwelling foreign-body ureteral stents.
