PROCEDURAL PATHWAY AT-A-GLANCE
Indications, Surgical Technique & Recovery
1. WHO IT'S FOR
• Severe refractory ED with limited hand dexterity / arthritis • Severe neurological impairment (stroke, spinal cord injury) • Phalloplasty or neopenis structural rigidity maintenance
2. HOW IT'S DONE
• Penoscrotal incision and corporal dilation • Semi-rigid titanium/silver core cylinder sizing & insertion • Zero fluid mechanics (no pump or reservoir)
3. RECOVERY
• Same-day outpatient discharge • Post-op dressing removal at 48 hours • Intercourse permitted at 4–6 weeks
A reliable prosthetic alternative utilizing two semi-rigid, positional cylinders implanted into the erectile chambers. This approach requires no fluid transfer components or manual pumping dexterity, offering an excellent, highly durable solution for patients seeking immediate physical rigidity with simplified management mechanics.
Clinical Indication
This procedure is highly indicated for patients experiencing structural anomalies, severe localized scar tissue, or functional blockages that fail to resolve with initial conservative therapeutic adjustments.
Surgical Objective
The principal objective is the complete restoration of physiological form, functional urinary path independence, or mechanical integrity, maximizing long-term outcomes while minimizing systemic downtime.
The Malleable (Semi-Rigid) Penile Prosthesis consists of two solid, bendable cylinders placed inside the corpus cavernosum. Each cylinder features a flexible core of braided silver or titanium wire wrapped in medical-grade silicone. Because the MPP contains no fluid, pump, or reservoir, it offers zero mechanical breakdown risk and extreme simplicity: the man simply bends the penis upward for intercourse and bends it downward for concealment in everyday clothing.
Clinical Indications & Patient Selection
An excellent alternative for men with refractory erectile dysfunction who are not optimal candidates for a 3-piece inflatable device.
Impaired Manual Dexterity / Arthritis: Patients with severe arthritis, Parkinson’s disease, or stroke who lack the fine motor finger strength to operate a scrotal deflation pump.
Hostile Abdominal / Pelvic Anatomy: Patients with extensive prior pelvic surgery, pelvic radiation, renal transplants, or multiple mesh repairs where reservoir placement carries higher risk.
Severe Neurological ED: Spinal cord injury patients who benefit from an MPP both for sexual activity and to facilitate external condom catheter placement.
Surgical Technique & Procedural Nuance
Performed through a small ventral penoscrotal incision under regional spinal or general anesthesia.
Corporal Dilation: Careful sequential dilation of the proximal and distal corpora cavernosa using Brooks dilators.
Precise Sizing & Trimming: The semi-rigid cylinders are measured using a Furlow gauge and customized with rear-tip extenders for an exact fit against the pubic bone.
Capsule Closure: The corporotomies are closed with running 2-0 absorbable sutures, followed by multi-layer closure of dartos and skin.
Postoperative Pathway & Recovery Timeline
Conducted as a rapid same-day outpatient procedure with minimal postoperative monitoring.
Dressing Removal: Compression wrap removed at 48 hours; mild scrotal elevation and icing.
No 6-Week Learning Curve: Because there is no pump to cycle or unlock, the patient only needs to wait for incision healing (4 weeks) before resuming sexual activity.
Return to Daily Life: Most men resume driving and light activities within 3 to 5 days.
Outcomes, Durability & Clinical Evidence
Provides exceptional long-term reliability with virtually non-existent mechanical failure rates.
Durability: No fluid or pump, so there is nothing to leak or fail mechanically.
High Patient & Partner Satisfaction: Satisfaction is high among men who value simplicity.
