Penoscrotal webbing is a structural condition where a redundant fold of scrotal skin extends abnormally far up the underside of the penile shaft. This creates a continuous 'web' or tether of skin that bridges the penis and scrotum together. This condition can be congenital or acquired following adjustments from a previous circumcision. While the internal penile anatomy is completely healthy, the tethered skin grid visually shortens the appearance of the penis and can create uncomfortable physical tension during erections.

Primary Symptoms

  • Webbed Profile: A distinct fold of scrotal skin that pulls upward onto the underside of the penis, obscuring the natural angle where the shaft meets the scrotum.

  • Altered Penile Appearance: The penis appears visually shorter than its true physical length because the underside is webbed.

  • Erectile Tightness: A localized feeling of skin stretching, pulling, or minor discomfort along the underside of the shaft during an erection.

Diagnostic Approach

  1. Visual Tissue Assessment: A complete clinical evaluation to measure the height of the webbed skin fold and check its elasticity.

  2. Functional Planning: Evaluating the skin availability surrounding the base of the penis to plan an optimal, geometric tissue rearrangement (such as a Z-plasty) to safely release the web.

Surgical Reconstruction: Complex Scrotoplasty — Z-plasty or V-Y advancement releasing high-riding scrotal skin to restore normal penoscrotal angle.
