PROCEDURAL PATHWAY AT-A-GLANCE

Indications, Surgical Technique & Recovery

1. WHO IT'S FOR

• Penoscrotal webbing ('turkey-neck') compromising visible length • Massive scrotal lymphedema, hydrocele burden, or laxity • Post-Fournier's gangrene or trauma tissue loss reconstruction

2. HOW IT'S DONE

• Z-plasty or V-Y advancement flap tissue transposition • Dartos muscle debulking and anatomical repositioning • Subcutaneous drainage and precise layered aesthetic closure

3. RECOVERY

• Outpatient day surgery • Scrotal elevation and supportive garment for 2 weeks • Strenuous physical exertion avoided for 3–4 weeks

A precise reconstructive contouring procedure designed to address structural anomalies of the scrotum, such as severe penoscrotal webbing or a bifid layout. Scrotal skin planes are geometrically rearranged to re-establish a natural anatomical transition, eliminating painful physical pulling or tethering tension.

Clinical Indication

This procedure is highly indicated for patients experiencing structural anomalies, severe localized scar tissue, or functional blockages that fail to resolve with initial conservative therapeutic adjustments.

Surgical Objective

The principal objective is the complete restoration of physiological form, functional urinary path independence, or mechanical integrity, maximizing long-term outcomes while minimizing systemic downtime.

Complex Scrotoplasty encompasses specialized plastic-reconstructive procedures of the scrotum and penoscrotal junction. It is designed to correct functional, anatomical, and aesthetic deformities including penoscrotal webbing (‘turkey-neck’ deformity), massive scrotal lymphedema, high scrotal insertions, and tissue loss following Fournier’s gangrene or scrotal trauma. Re-establishing a defined 90-degree penoscrotal angle immediately restores perceived penile shaft length and eliminates scrotal trapping.

Clinical Indications & Patient Selection

Indicated for men experiencing functional discomfort, cosmetic distress, or loss of visible penile length due to scrotal positioning.

Penoscrotal Webbing (Turkey-Neck): Scrotal skin web inserting high onto the ventral penile shaft, visually burying flaccid length and causing condom displacement.

Massive Scrotal Lymphedema / Hydrocele Laxity: Severe chronic skin stretching from lymphedema, large neglected hydroceles, or scrotal elephantiasis.

Post-Infectious / Trauma Reconstruction: Scar contracture, asymmetry, or tissue loss requiring flap advancement following Fournier’s gangrene debridement.

Surgical Technique & Procedural Nuance

Utilizes meticulous geometrical plastic surgical flap design to avoid linear contracture bands.

Z-Plasty & V-Y Advancement Flaps: Geometric transposition of skin flaps releases longitudinal tension, drops the scrotal attachment, and deepens the penoscrotal angle.

Dartos Muscular Suspension: Debulking of excessive dartos fascial fat pads and secure anchoring of the scrotal base to the underlying tunica dartos and pubic fascial plate.

Multi-Layer Tension-Free Closure: Subcuticular dissolvable suture closure eliminates stitch marks and ensures a smooth, natural scrotal contour.

Postoperative Pathway & Recovery Timeline

Conducted as a same-day ambulatory outpatient surgical procedure under light sedation or local anesthesia.

Scrotal Support & Elevation: Patients wear a specialized scrotal supporter or snug briefs with ice packs for the first 72 hours.

Activity Restrictions: Avoid strenuous lower-body exercise, squatting, and heavy lifting for 3 to 4 weeks.

Healing Timeline: Full aesthetic tissue settling occurs over 4 to 6 weeks.

Outcomes, Durability & Clinical Evidence

Scrotoplasty delivers immediate, dramatic improvements in masculine genital contour and comfort.

Restored Visible Penile Length: Reveals 1 to 2 inches of previously concealed ventral penile shaft, dramatically improving flaccid appearance.

Enhanced Sexual Comfort: Eliminates scrotal pinching, skin irritation during exercise, and condom slippage.
