PROCEDURAL PATHWAY AT-A-GLANCE

Indications, Surgical Technique & Recovery

1. WHO IT'S FOR

• Stable-phase Peyronie’s disease (<60° simple curvature) • Congenital penile chordee without hypospadias • Preserved baseline erectile rigidity

2. HOW IT'S DONE

• Degloving or 16-dot Lue minimal-dissection approach • Non-absorbable polytetrafluoroethylene (Gore-Tex) plication • Artificial erection testing for real-time straightness check

3. RECOVERY

• Outpatient day surgery (1–2 hour procedure) • Light compressive dressing for 48 hours • Sexual abstinence for 6 weeks during scar healing

A precise structural tailoring procedure designed to correct significant curvature caused by Peyronie's disease or congenital chordee. Permanent architectural sutures are placed on the side opposite the curvature to accurately level the lengths of the structural chambers, straightening the anatomy without tissue grafts.

Clinical Indication

This procedure is highly indicated for patients experiencing structural anomalies, severe localized scar tissue, or functional blockages that fail to resolve with initial conservative therapeutic adjustments.

Surgical Objective

The principal objective is the complete restoration of physiological form, functional urinary path independence, or mechanical integrity, maximizing long-term outcomes while minimizing systemic downtime.

Penile plication is a minimally invasive reconstructive procedure for correcting penile curvature caused by stable-phase Peyronie’s disease or congenital chordee. By placing durable, non-absorbable sutures in the tunica albuginea on the convex side (opposite the plaque or curvature), the elongated side is shortened to match the shorter side, restoring an aesthetically straight erect shaft without grafting or entering the erectile cavernosal spaces.

Clinical Indications & Patient Selection

Indicated for men with stable-phase Peyronie’s disease or congenital chordee who possess adequate baseline erectile rigidity.

Stable Peyronie’s Disease: Plaque has matured for at least 12 months with stable curvature and complete resolution of painful erections for >3–6 months.

Moderate Curvature (<60°): Curvature without severe hinge-effect instability, hourglass narrowing, or significant baseline erectile dysfunction.

Congenital Chordee: Curvature present since adolescence without hypospadias or urethral abnormality.

Surgical Technique & Procedural Nuance

Performed via a circumcising coronal incision or small 1–2 cm longitudinal counter-incisions (Lue 16-dot minimal dissection technique).

Artificial Erection Test: Saline is injected into the corpus cavernosum through a butterfly needle with a vascular clamp at the base to accurately measure baseline curvature and rotation.

Tension-Free Plication Sutures: Non-absorbable 2-0 or 3-0 braided polyester or PTFE sutures are placed in the outer layer of the tunica albuginea opposite the curve.

Real-Time Straightness Verification: Repeat artificial erection verifies complete straightening (<10° residual) with knot burial to prevent palpable suture nodules.

Postoperative Pathway & Recovery Timeline

Performed as a same-day outpatient procedure lasting approximately 60 to 90 minutes.

Compressive Dressing: A light Coban compressive dressing remains for 48 hours to minimize bruising and swelling.

No Foley Catheter Required: Patients void normally immediately after recovery and are discharged home.

Sexual Abstinence: Strict avoidance of sexual intercourse and masturbation for 6 weeks to ensure suture security.

Outcomes, Durability & Clinical Evidence

Penile plication provides the highest safety profile of any penile straightening procedure with virtually zero risk of new erectile dysfunction.

Straightening Success: Most men achieve an erection straight enough for intercourse.

Preserved Sensation & Potency: Low risk of new erection problems or numbness, because the nerves are protected during the repair.
