Trimix Penile Injections for ED
What Trimix is
Trimix is a compounded mix of three medications (papaverine, phentolamine, and alprostadil) injected with a very fine needle into the side of the penis. It acts directly on the erectile tissue, so it often works when pills don’t, including after prostate cancer treatment or with diabetes.
Who it’s for
Men whose erections don’t respond well enough to pills, who can’t take them, or who don’t tolerate the side effects. It’s also a reasonable option to try before considering a penile implant.
Your first dose is in the office
We do the first injection together in the office to find the right dose and teach the technique: drawing up the dose, choosing the spot, and injecting. You leave with written instructions and a video to review at home.
Using it at home
An erection usually starts within 5 to 15 minutes. Use it no more than once in 24 hours, alternate sides, keep it refrigerated, and use only the dose you were prescribed.
Safety
The main risk is an erection that lasts too long (priapism). It’s uncommon and treatable when handled early: if you’re still fully hard at 2 hours, call the office; by 4 hours you should be at the office or an emergency room. The home guide has the full plan. Minor bruising can happen; alternating sides helps prevent scarring.
Cost
Trimix is made by a compounding pharmacy and is often not covered by insurance. Alprostadil on its own (Caverject or Edex) is an FDA-approved alternative that some plans cover.
