PEYRONIE’S DISEASE

Xiaflex Injections for Peyronie’s Disease

What Xiaflex is

Xiaflex (collagenase clostridium histolyticum) is the only FDA-approved medication for Peyronie’s disease. It is an enzyme injected into the plaque that breaks down some of the scar tissue causing the curve. I give it in the office in Garden City and am listed among high-volume Xiaflex urology specialists.

Who it’s for

Men whose curve has stopped changing, with a plaque that can be felt and a curve of 30 degrees or more, which is what the FDA approval covers. It isn’t used during the early, painful phase while the curve is still changing. An exam and a measurement of the curve come first.

What a treatment course looks like

A course is up to four cycles. Each cycle is two short office injections a few days apart, followed by gentle stretching and straightening at home and in the office. Cycles are spaced about six weeks apart, and we reassess the curve along the way.

Side effects and safety

Bruising, swelling, and tenderness are common and usually settle within a week or two. The serious risk is a penile fracture, a tear in the erectile tissue, so avoid sexual activity for at least two weeks after the second injection of each cycle. Call right away for a popping sound with sudden loss of erection, rapidly spreading bruising, or trouble urinating.

Xiaflex or surgery?

Xiaflex aims to reduce the curve without surgery. If the curve is severe or complex, such as an hourglass or hinge deformity, or comes with erection problems, plication, grafting, or a penile implant with straightening may be the better route. We go through every option at your visit.

Insurance

Xiaflex usually needs prior authorization from your insurance plan, and coverage varies. We start that process after your visit.

Traction therapy

Traction is often used alongside Xiaflex to help hold the gains. The home traction guide is under Patient Guides.
